    As a courtesy to our patients we may file your claim, however the patient is responsible for charges incurred from the date services are provided, unless our contractual agreement with your carrier states otherwise.

    Health plans change constantly, so it is imperative that you are familiar and understand your benefits and responsibilities before your visit.
